Who Isn't a Candidate

Men actively trying to conceive, men with untreated prostate or breast cancer, men with untreated severe sleep apnea, or men with a hematocrit above 50 percent. Dr. Arshad screens for all of this first.

When it's prescribed and monitored by a physician for men with genuinely low testosterone, TRT has a strong safety record. The landmark TRAVERSE trial, published in 2023 and involving over 5,200 middle-aged and older men with cardiovascular risk, found no increased risk of heart attacks, strokes, or cardiovascular death compared to placebo. Based on those findings, in February 2025 the FDA removed the long-standing boxed warning about cardiovascular risk from testosterone product labels. That said, TRT is a prescription therapy that requires ongoing lab monitoring, which is exactly how we run it here.
The most common side effects are manageable and usually respond to a dose adjustment. They include elevated red blood cell count (hematocrit), acne or oily skin, mild fluid retention, breast tenderness if estradiol runs high, and reduced sperm production (which matters if you're trying to conceive). Less common: worsening sleep apnea, mood swings in men who overshoot the dose, and mild hair shedding in genetically predisposed men. Regular lab monitoring catches these early and lets us adjust before they become a problem.
A full panel at baseline, then a follow-up at 3 months and 6 months after starting therapy. After that, every 6 to 12 months for as long as you're on TRT. We track testosterone, hematocrit, PSA, estradiol, lipids, and a few other markers to keep everything in a healthy range.
This was the biggest question hanging over TRT for years, and we finally have a clear answer. The TRAVERSE study, the largest randomized TRT safety trial ever conducted, followed men at elevated cardiovascular risk for an average of 33 months and found no increase in major cardiovascular events on testosterone compared to placebo. The FDA officially removed the cardiovascular boxed warning from all testosterone products in February 2025 based on those findings. TRT can raise blood pressure modestly and can raise red blood cell count, both of which we monitor closely, but the old fear of testosterone causing heart attacks is no longer supported by the evidence.
For most men, TRT is a long-term therapy. Your body won't suddenly start making more testosterone on its own once you start replacing it. As long as you feel great and your labs look clean, there's no set stop date. If life circumstances change or you want to come off, we can taper you down gradually. Some men choose to pause for fertility reasons, and we have protocols to help the body restart its natural production.
